Promote engagement of the patient and family to improve patient health and self-management
Focus on primary care prevention and early diagnosis, through improvement in identified measures
Focus on effective use of clinical decision support, clinical quality improvement, and using the Electronic Health Records EHR to track and improve population health and the identified clinical targets

Goal:

6% relative improvement from baseline

Flu immunizations of patients ages 50 and older during the flu season
Pneumococcal immunization of patients ages 65 and older
Appropriate low-dose aspirin therapy use in patients with ischemic vascular disease
Blood Pressure (BP) control in patients with hypertension
LDL-C control among adults with ischemic vascular disease
Tobacco cessation intervention among adult patients who smoke (screening and cessation counseling)
Colorectal cancer screening in patients ages 50-75
Breast cancer screening in women ages 40-69
