Goals:

Educate senior patient population generally Medicare Beneficiaries to better understand:

  • Hospital discharge instructions to reduce readmissions
  • Medication instructions to reduce adverse drug events and progression of uncontrolled diseases
  • Personal health information to improve healthcare outcomes


Actions needed to achieve these goals:

  • Building community awareness through (outreach activities and media messages)
  • Educating providers of the importance of health literacy and the use of plain language
  • Recruiting high-risk patients -Population of Focus (PoF) for health literacy trainings
  • Recruit Medicare patients and other seniors for health literacy trainings
