LAN - Reducing Healthcare Associated Conditions (HACs) Network
Description
Goals:
- Reduce the rate of falls among nursing home resident by identifying the underlying causes of falls in nursing home patients and promoting best practices to reduce falls.
- Improve nursing home documentation by promoting sharing best practices and tools to improve documentation; and assessing the current level of documentation through chart abstractions
- Improve nursing home staff stability by promoting the incorporation of consistent assignments in the nursing home
Measure(s):
- Rate of improvement in falls (40% relative reduction)
- Consistent Assignment of CNAs in the nursing home (85% improvement)
- Improvement in ADMIN/RN/LPN/CNA turnover in the nursing home from the prior year (60% improvement)
